UCSF Professional Liability (PL) Insurance Proof of Coverage & Certificate of Insurance (COI) Info & Requests

Please note that proof of Professional Liability coverage and a UCSF COI are not the same thing. In the University of California world a COI has a very different meaning from a proof of PL insurance coverage letter, they are two very different things whereas at most other institutions they are one and the same. If you will continue to be a UCSF employee working at a non-UCSF facility, then you need a COI. If you are a current or former UCSF employee applying for a new job at a non-UCSF facility (you are or will no longer be a UCSF employee), then you need the proof of PL insurance coverage letter.

UCSF Professional Liability (PL) Insurance Proof of Coverage:

PL coverage is provided through the UC Regents self-insurance program. It is provided to all UCSF employees when working within their course and scope of employment or training program with UCSF. We do not provide PL coverage for moonlighting or private practice work. 

1) What is the name and address of the PL coverage?

2) What are the coverage dates?

Coverage is concurrent with dates of employment from the first day of employment/attendance to the last day at UCSF. If you need these dates, please click on this link Employee & Student Verification and follow the instructions. UCSF Health Risk Management does not provide these dates.

3) Is there a policy number, face sheet, or certificate for the UCSF PL insurance?
  • There is no policy number, face sheet, or certificate for coverage. The reason for this is that UC Regents owns their own medical insurance under the California Tort Claims Act, section 810. For more information please click on this link: UCOP BUS 81
  • A letter verifying coverage may be requested by contacting the UCSF Risk Management campus the provider attended/worked at. Please see our different locations by clicking on this link and scrolling to #10: Claim History & Proof of Professional Liability Insurance Requests
  • If you are filling out an online application, you can enter in "00000000" or "N/A" for the policy number section.
4) What is the coverage amount?

Coverage always meets the industry standard, currently $1,000,000 per occurrence; $3,000,000 aggregate.

5) Is Tail coverage provided? Is my PL coverage claims based or occurrence based?

UCSF's insurance coverage is occurrence based, so there is no need for Tail coverage. Occurrence based policy: the incident must have happened while the policy was active. It offers lifetime coverage for incidents that occur during the policy period regardless of when the claim is reported. Claims based policy only covers incidents that occur and are reported within the policy's time frame; therefore, a Tail extension coverage would be needed.

6) How do I get proof of PL coverage?

UCSF Certificate of Insurance (COI): 

A COI is issued to UCSF healthcare providers who are working at non-UCSF facilities (ex., Stanford Medicine, UCSF Benioff Children's Hospital Oakland, John Muir Health, Sutter Health, etc.). Once a COI is issued a provider will then have coverage with the UCSF self-insurance program UC Regents, as long as their work is within their course and scope of employment at these non-UCSF locations. We do not provide COIs for moonlighting or private practice work. A contract or Provider Service Agreement (PSA) between UCSF Health and the non-UCSF facility must be in place for a COI to be issued.

1) How to get a copy of a UCSF provider's COI?

The provider will be able to obtain a copy of their COI from the UCSF department they work in. Each department has a designated person who handles COI's, this information is listed on the UCSF Health Risk Management internal website (employee only access via CareLinks or MyAccess) here COI Requestors. UCSF Health Risk Management does not provide copies of COIs.

2) How does a UCSF provider get a COI to work at non-UCSF facilities?

The provider will need to have their UCSF department request the COI on their behalf. Details are located here on the UCSF Health Risk Management internal website (employee only access via CareLinks or MyAccess) - COI application.
