Note: UCSF Health Risk Management is not an Agent for service of process for Summons and Complaints, please see #3 below for details. 

UCSF Health Risk Management only accepts subpoenas for personal appearance at deposition, trial/hearing, or arbitration for our employed providers/staff. We do not accept subpoenas for non-UCSF providers (not all providers at a UCSF facility are UCSF employees). 

We do not accept subpoenas for personal or non-UCSF business matters. We do not accept subpoenas for the production of any documents or records, please see #1 below for details.

1) How to Obtain Medical, Financial, HR Records (UCSF Health Risk Management does not handle these). 

a) To serve a subpoena to UCSF Dental Records please follow the instructions in this how to guide.

b) To serve a subpoena to UCSF Radiology (imaging records only, contact HIMS department for all other Radiology medical records) please follow the instructions in this how to guide.

c) To serve a subpoena to UCSF Pathology please follow the instructions in this how to guide.

d) To serve a subpoena to UCSF HIMS (patient health medical records) please follow the instructions in this how to guide

e) To serve a subpoena to UCSF Patient Financial Services (patient billing records) please follow the instructions in this how to guide.

f) To serve a subpoena to UCSF Human Resources (employee HR records) please follow the instructions in this how to guide.

2) How to Serve a Subpoena for Personal Appearance at a Deposition, Trial/Hearing, or Arbitration

UCSF Health Risk Management only accepts Subpoenas for personal appearance for current UCSF providers and staff. We no longer accept personal service of Subpoenas via Process Servers. Subpoenas should be emailed to [email protected] or sent via postal carrier, please see #6 below for details.  For Subpoena questions please call (415) 353-1842 option 3.

As of August 1, 2024, UCSF Health acquired St. Mary's Hospital and Saint Francis Hospital. These are community hospitals and have an open medical staff inclusive of community physicians and UCSF Health employees. Therefore, before sending us a Subpoena for a provider, please double check that they are a UCSF Health employee.

3) How to Serve a Summons and Complaint (UCSF Health Risk Management does not accept these).

A Summons and Complaint which names UC Regents must be served at the University of California Office of the President (UCOP) in Oakland, California. Pleae click this link and read section # 1 on how to serve UCOP. UCSF Health Risk Management will receive a copy of the Summons and Complaint from UCOP. If a Summons and Complaint names a healthcare provider as an individual defendant, that provider must be personally served with the documents by a Process Server. 

4)  How to Request Expert Witness Review from a UCSF Provider. 

The decision to serve as an expert witness in litigation is at the discretion of the individual UCSF healthcare provider. Please reach out directly to the provider's practice / office. UCSF Health Risk Management will never provide healthcare providers' contact information, their contact information will often be listed online. Advise the office when calling that you seek to retain the provider as an expert witness.  

5) How to Send Workers' Compensation Claims (UCSF Health Risk Management does not accept these).
