Please carefully read this list before filling out the Patient Lost Property online form below.

UCSF Health Risk Management is not Lost & Found. For UCSF Health Lost & Found, please contact UCSF Health Security, T: (415) 885-7890. 

1) Patients are discouraged from bringing valuable items to the hospital. UCSF Health is not responsible for personal belongings and valuables kept by a patient during their stay or procedure. Please note that after evaluation, reimbursement may not be provided for all lost items. 

2) UCSF Health does not reimburse for lost electronic devices, however please fill out the Patient Lost Property form in #2 below. We may be able to help locate your lost electronic device.

3) If you believe that your property was stolen, please immediately call UCPD at (415) 476-1414.

If you would like to file a claim for lost property please see below:

1) If you are currently a patient at UCSF:  please immediately notify your UCSF nursing staff that your property is missing. Ask them to follow the proper procedures on the employee only UCSF Health Risk Management website in notifying us of your lost property.

2) If you have been discharged from UCSF:  please first contact the hospital unit where your items were lost. After talking with the unit and if they are not able to find your items, then please "click on this link" to fill out the form for your lost item(s).  After the online form is submitted, our office will review and research with the unit what may have happened. Our average turn around time for this is 10 business days. Repeated form requests will not speed up the process. This request, does not guarantee reimbursement after the evaluation has been completed.

UCSF Health Risk Management will contact you if further information is needed. You will be notified when a determination has been made regarding reimbursement. If approved, please note that reimbursement checks can take 2 - 3 weeks to process.
