There is no policy number, face sheet, or certificate for coverage. A letter verifying coverage may be requested by contacting UCSF Health Risk Management.  Please see contact information below. If you are / were a UCSF provider filling out an online application you can enter in "00000000" or "N/A" for the policy number.

Coverage always meets the industry standard, currently $1,000,000 per occurrence; $3,000,000 aggregate.

Our insurance coverage is occurence based, so there is no need for Tail coverage. Occurence based policy: the incident must have happened while the policy was active. It offers lifetime coverage for incidents that occur during the policy period regardless of when the claim is reported. Claims based policy: only covers incidents that occur and are reported within the policy's time frame, therefore a Tail extension coverage would be needed.

The provider will be able to obtain a copy of their COI from the UCSF department they work in. Each department has a designated person who handles COI's, this information is listed on the UCSF Health Risk Management internal website (employee only access via CareLinks or MyAccess) here COI Requestors. UCSF Health Risk Management does not provide copies of COIs.
