UCSF Claim History & Proof of Professional Liability Insurance Requests

UCSF Health Risk Management will provide a Claim History & Proof of Professional Liability Insurance letter for any of our current or former employed providers (this includes Residents and Fellows). We do not provide claim histories for non-UCSF providers (not all providers at a UCSF facility is a UCSF employee) or for personal interest. The claim history will reflect any claim or litigation that arose during the provider's time at UCSF in which the provider was either a named party or the provider’s care and treatment were the subject of the claim/suit. Our claim history reporting goes back six years unless requested otherwise. The letter will also provide the levels of insurance coverage. The letter does not provide dates of employment or attendance; for that information, please see # 4 below.

1) If you were a former healthcare provider at UCSF:

Please contact the UCSF Risk Management office at the location(s) that you work/worked at in # 10 below.

2) What information is needed for a UCSF Claim History & proof of Professional Liability (PL) insurance request?

We need a verification and provider signed consent form to release a claims history and proof of PL insurance coverage request. When sending the verification and signed consent to release forms please include the provider's: NPI#, date of birth, full name (first, middle, last) and if the provider has multiple names, please list all of them (ex; maiden name, name change). UCSF does not use dates of birth or social security numbers to lookup providers and sometimes we have more than one provider with the exact same name therefore, the NPI# is the backup search source. 

3) The Healthcare Provider could not be located when the Claim History & Proof of Professional Liability insurance request was made, why?

Please note there are five University of California (UC) Health's Medical Centers each with their own Risk Management department: UC San Francisco (UCSF), UC Los Angeles (UCLA), UC Davis (UCD), UC Irvine (UCI), and UC San Diego (UCSD). Please make sure you are sending the request to the correct UC Medical Center. This is the most common mistake when sending a claim history request and will delay the process. UCSF Health Risk Management only accepts claim history and proof of professional liability insurance coverage requests for UCSF Health. UCSF Fresno falls under the UCSF umbrella, but they have their own Risk Management department, please see #10 below for their contact information.

4) What if employment or attendance dates are needed?

UCSF Health Risk Management does not provide employment or trainee (this includes GME, Residents, Fellows, and Attendees) attendance dates. Please refer to the Employee & Student Verification Process on how to get this information.

5) How do I get the provider's proof of Professional Liability (PL) insurance coverage and / or their Certificate Of Insurance (COI)? Please note, these are two very different items within the University of California (UC) system.

On the UCSF claim history letter, the provider's PL coverage will be explained. UCSF PL insurance is provided by the UC Regents self-insurance program; there is no policy number, certificate, or face sheet for individual providers. Please note that PL coverage is different from a COI. 6) What are the dates of Professional Liability coverage?

The dates of coverage coincides with the provider's first day of employment through their last day of employment at UCSF. For dates of coverage please see #4 above. UCSF Health Risk Management does not provide these dates.

7) What time frame does the UCSF Claim History & proof of Professional Liability insurance request cover?

The past six years. If it needs to go back further than six years, please enter the number of years on the UCSF Health platform as listed in #10 below. For our other Risk Management locations, write your request in the body of the claim history email you send to them.

8) When can I expect to receive a UCSF Claim History & proof of Professional Liability insurance letter?

UCSF Health's average turnaround time is 72 business hours

9) What if my UCSF Claim History & proof of Professional Liability insurance request is urgent?

10) Where and how to send your UCSF Claim History & proof of Professional Liability insurance requests (please double check where your provider worked or currently works at, and then send it to that location(s)):

Please check the list below to see where the provider is / was located before sending your request as we have multiple locations. If your provider worked at more than one of these locations, then please send it to all appropriate contacts. Please also check that your provider was a UCSF employee, as some of these locations hire non-UCSF employees. If you are uncertain as to where your request should go, please start with letter "a" below.
