UCSF Health Risk Management will provide a Claim History for any claim or litigation that arose from a provider's alleged professional liability if the provider was either a named party, or the provider's care was the subject of the claim and it was reported to us within the past six years. This same letter will provide Professional Liability (Malpractice) proof of coverage for that provider as well.
