Please also check that your provider was a UCSF employee, as some of these locations hire non-UCSF employees. If you are uncertain as to where your request should go, please start with letter "a" below.

UCSF Health Stanyan Hospital and UCSF Health Hyde Hospital are community hospitals so please make sure the provider is / was a UCSF Health employee during their time at those locations. UCSF BCH OAK is a community hospital so there will be UCSF Health and non-UCSF Health employees working there. Providers cannot request their own claim history reports.

D) For UCSF @ Zuckerberg San Francisco General Hospital (ZSFG) please send your claim history request to [email protected], please add in the email subject line "ZSFG". Providers cannot request their own claim history reports.
